Root canal treatment under a microscope

At UKRMED in Kyiv, a dental microscope is used for primary root canal treatment and for reopening canals filled in the past. The working area inside a tooth is tiny, so extra light and magnification matter. Before treatment, the endodontist reviews an X-ray or CT scan and checks whether the tooth can be restored afterwards.

Toothache is not always dramatic. One patient arrives after a sleepless night; another feels only slight pressure when biting. Occasionally, nothing hurts. A routine image simply reveals a change beside the root.

At the centre of the tooth is a chamber containing the dental pulp — nerves, blood vessels and connective tissue. Deep decay may open a route into it. So can trauma, a crack or the loose edge of an old filling. Once bacteria enter, the pulp can become inflamed or infected.

Root canal therapy removes the damaged tissue, disinfects the internal space and seals it. The aim is to retain the natural tooth when the root is sound and enough hard tissue remains for restoration.

A closer look inside the tooth

Canal anatomy is rarely convenient. A passage may curve, divide near the end or narrow until it is difficult to distinguish from the surrounding dentine. A molar can also contain an additional entrance hidden in a groove on the floor of the pulp chamber.

A dental microscope brings light directly into this area. Under magnification, the endodontist can distinguish natural tissue from old filling material, search for a missed canal and inspect thin walls more carefully.

It is especially useful during root canal retreatment. Gutta-percha, cement, a post or a separated instrument may already occupy the route. Removing these materials without a clear view risks taking away sound dentine.

Not every obstacle should be removed at any cost. If an instrument fragment lies deep in a curved root, trying to retrieve it may weaken or perforate the canal wall. The dentist first weighs that risk against the likely benefit.

What brings patients to an endodontist

People rarely describe this pain in the same way. Hot tea may start a dull ache that hangs around. Biting into firm food might hit one exact spot. In quieter cases, there is no obvious pain — the tooth simply seems to meet its opposite before the others do.

Changes on the gum are easy to dismiss. A small pimple-like mark can appear, drain and vanish, only to return later. A tooth that suffered a knock months ago may also begin to darken after the initial soreness has passed.

The appointment is partly detective work. Cold is applied; the tooth is tapped and the bite checked. An image shows what is happening around the root, but it must match the clinical findings. Otherwise, discomfort from a crack, an overloaded contact or a neighbouring tooth could be mistaken for infected dental pulp.

Deep decay and trauma are frequent causes. Old treatment can be another: bacteria sometimes return beneath a leaking restoration, or inflammation remains because part of the canal system was never reached.

How microscope-assisted treatment is carried out

First comes the question of whether the tooth can be rebuilt. A deep vertical root fracture or destruction far below the gum may leave little chance of a lasting result, however thoroughly the canals are cleaned.

Local anaesthesia is given. The tooth is then isolated with a rubber dam — a thin elastic sheet that keeps saliva away and protects the mouth from disinfecting liquids.

After access has been created, the endodontist locates the canal entrances under magnification. Working length is checked with an electronic apex locator and, where necessary, a control X-ray.

Fine instruments shape the canals. Irrigating solutions wash through smaller spaces that a file cannot physically enter. Once the passages are clean and dry, root canal filling material seals the prepared internal space.

Some cases are finished in one appointment. Active infection, difficult anatomy or retreatment may require several visits, with medication and a temporary filling placed between them.

Root canal retreatment

A treated tooth can develop problems again. Perhaps one canal was missed. The old filling may stop short of the root end, or bacteria may have entered beneath a crown that lost its seal.

Retreatment starts with removing the previous material. The endodontist then searches for untreated areas and decides whether the cause can be addressed without taking away too much root tissue.

This work is often slower than primary treatment, particularly when a post or dense cement blocks the route. The microscope does not guarantee success, but it makes the reason for earlier failure easier to investigate.

Restoring the tooth afterwards

Filling the canals is not the final stage. The visible part of the tooth still has to withstand chewing, while its walls may already be thin after decay, previous fillings and the access opening.

Composite may be sufficient when the defect is limited. A heavily damaged molar is different: its remaining cusps can flex and crack, so a ceramic inlay or crown may be recommended.

The decision is based on what remains above the gum and the load carried by that tooth. Good endodontic treatment protects the root from infection; a suitable restoration protects the crown from fracture.

Root canal treatment cost in Kyiv

Microscope-assisted treatment of one root canal at UKRMED costs UAH 1,800. Reopening a previously filled canal is UAH 1,300, removal of a foreign object from a canal is UAH 1,400, and microscope use as a separate treatment stage is UAH 1,200.

A dental consultation costs UAH 500 and an X-ray costs UAH 250. The remaining amount depends on the number and shape of the canals, old filling material and the condition of the tooth. Anaesthesia, rubber dam isolation, canal preparation, filling and the final restoration may be included as separate items in the treatment plan.
