The 2017 clinical practice guideline of the American Academy of Otolaryngology–Head and Neck Surgery (AAO-HNS) stresses that nasal breathing must be assessed before rhinoplasty, as changing the shape can improve or worsen it. With congestion, an old fracture or a deviated septum (and, for complex post-traumatic deformities, the midface bones), surgical nose correction combines the cosmetic part with septoplasty.
Tip refinement is the most delicate part of the operation: the surgeon works with cartilages that determine width, projection and the angle between nose and lip. Over-weakening them can worsen breathing, so support is preserved or reinforced. Bridge work covers humps, bony width, depressions or a crooked line.
Primary rhinoplasty is surgery on a nose that has not been operated on. Revision rhinoplasty is more complex: scar tissue changes the layers, less native cartilage remains, and cartilage from other areas is sometimes needed. It is planned only after the tissues have fully healed from the first operation.
The consultation is a key step. The surgeon photographs the nose from several angles and asks about injuries, previous surgery, illnesses, medicines and breathing. The AAO-HNS guideline recommends discussing realistic expectations and watching for signs of body dysmorphic disorder — an excessive focus on a perceived flaw — in which case surgery does not bring satisfaction. Cosmetic rhinoplasty is planned once facial growth is complete.

Nasal plastic surgery is performed under general anaesthesia supervised by an anaesthesiologist. The approach is closed — through incisions inside the nose — or open, with an extra incision across the columella between the nostrils. The surgeon reshapes bone and cartilage, uses your own cartilage if needed, and places internal supports and an external splint at the end.
In the first weeks, swelling, bruising under the eyes, congestion and slight blood-stained discharge are possible. The splint and supports are removed on the surgeon's schedule. The AAO-HNS guideline advises explaining the expected recovery in advance: most swelling settles within weeks, while the final contour, especially of the tip, forms over up to a year. Do not press on the nose, sleep with your head raised, and avoid exertion and glasses resting on the bridge until the surgeon allows.
Bleeding, infection, reaction to anaesthesia, prolonged swelling, altered sensation, asymmetry, breathing problems and the need for revision. A visible scar on the columella after an open approach is rare and can be improved with scar revision. Surgery is postponed with acute infection, uncontrolled illness or bleeding disorders. Always tell the surgeon about smoking and all medicines.
