Tooth Extraction in Kyiv

Tooth removal is the last resort, used when a tooth cannot be saved by treatment. At UKRMED Dentistry, the decision to extract is made only after an X-ray or CT scan and an attempt to keep the tooth. The procedure is done under modern anaesthesia, so painless tooth extraction is the standard, not a bonus. A simple tooth extraction takes 15–20 minutes, a surgical tooth extraction up to an hour.

When a tooth needs to come out

Modern dentistry tries to save every tooth: microscope-assisted root canal treatment, apicoectomy, posts and crowns rescue even badly damaged teeth. Dental extraction is considered when:

• the root has a vertical crack or fracture;
• the tooth is broken below the gum and cannot be restored;
• inflammation at the root does not respond to treatment;
• the tooth is very loose due to bone loss from periodontitis;
• a wisdom tooth is badly positioned and causes inflammation;
• extraction is needed for orthodontic treatment.

Types of extraction

• Simple extraction — a single-root tooth with an accessible crown, loosened with forceps and removed whole.
• Complex extraction — multi-root teeth, curved roots or a broken crown. The tooth is sectioned and removed piece by piece.
• Broken tooth extraction — when the crown has crumbled and only the root remains in the bone (tooth root extraction).
• Surgical extraction — an unusual position or anatomy, requiring a gum incision and access through bone.
• Wisdom tooth removal — often impacted. See wisdom tooth removal under sedation and removal of impacted teeth.

The procedure

1. Diagnosis. The dentist examines the tooth and orders a periapical X-ray or CT to assess the roots, nerve and maxillary sinus.
2. Anaesthesia. Local anaesthetic works within minutes: you feel pressure but not pain.
3. Extraction. The gum is detached and the tooth loosened or sectioned, carefully preserving the bone.
4. Socket care. The socket is cleaned, a haemostatic sponge or your own plasma concentrate (PRGF) is placed if needed, and the gum is stitched.
5. Instructions. The dentist explains how to care for the socket and books a check.

For very anxious patients, extraction can be done under sedation supervised by an anaesthesiologist.

Contraindications and what to tell the surgeon

Planned extraction is postponed during acute infections, in the first months after a heart attack or stroke, and with some blood disorders. During pregnancy, treatment is planned in agreement with your doctor.

Always tell the surgeon if you take blood thinners — do not stop them on your own. Special care is needed for patients on bisphosphonates or other antiresorptive drugs, or who have had radiotherapy to the jaws: the 2022 position paper of the American Association of Oral and Maxillofacial Surgeons (AAOMS) describes the risk of osteonecrosis of the jaw after extraction in such patients, so the surgeon plans additional measures.

Emergency tooth extraction

Severe pain, facial swelling and fever are signs of a spreading infection. Emergency tooth extraction is carried out on the day you come in if the tooth cannot be saved. With an abscess, the focus is first opened and drained — see oral surgery.

Aftercare

• bite on a gauze pad for 20–30 minutes;
• apply a cold pack to the cheek for the first hours;
• do not rinse vigorously for the first day, so the clot stays in place;
• do not smoke or drink through a straw;
• eat soft food on the other side.

The most common complication is dry socket, an inflamed socket after the clot breaks down, causing severe pain two to four days later. A 2026 meta-analysis showed that autologous platelet concentrates reduce the risk of dry socket after lower wisdom tooth removal by about two-thirds. If pain increases, contact the surgeon: dry socket is treated by rinsing and a medicated dressing.

Extraction before an implant

A missing tooth should be replaced: otherwise neighbouring teeth tilt and the bone shrinks. Sometimes an implant is placed straight into the fresh socket. If not, the surgeon preserves the bone walls and may fill the socket with graft material — see bone grafting.
